We’re so glad your family is joining us in Fusion Kids! We’re excited to welcome your child and want to make sure their experience in Fusion Kids is the best part of their week.

Before your child can be checked in, we ask all first-time families to complete this Family Information Form.

This helps us:
  • Ensure your child’s safety and care
  • Have accurate contact and emergency information
  • Provide a secure and efficient check-in and check-out process
This form is quick and should only take a few minutes to complete. Be sure to add everyone in your household, and birthdays are important to help make sure kids join the correct class.
